Hospice and Palliative Care RN

Position Overview:

The Hospice and Palliative Care RN’s primary responsibility is to grow the palliative and hospice program census by identifying, coordinating and maximizing their care within the All Care continuum. This position will work closely with the VNA program both internally and externally as well as other referral sources to strengthen partnerships that will foster a continuous flow of referrals for the growth of the company. The position will provide support to patients and families with transitional care needs across the All Care continuum. Through disease stage changes, goals of care discussions and coordination with patient’s primary medical team the Hospice and Palliative Care RN will transition patients to hospice or palliative care.

Essential Duties & Responsibilities:
  • Ensure the growth of the hospice census, exceeding budget goals for the hospice program.
  • Ensure the growth of the palliative care census exceeding budgeted goals for the palliative program.
  • Provide hospice and/or palliative informational visits as necessary
  • Collaborate with referral sources with effective communication and education with the goal to grow referrals to the hospice and palliative care programs.
  • Attend required meetings with hospice and VNA teams to help identify and drive hospice and palliative referrals and assist with coordinating services.
  • Provide education to VNA teams as needed.
  • Review data extractions for hospice and high risk patients
  • Provide training and continuing education to referral sources as needed.
  • Respond to inquiries regarding care services and programs to accurately identify the needs of each patient.
  • Screen all VNA, hospice, and palliative care referrals and identifies appropriate Palliative care and hospice patients.
  • Act as a clinical resource to coordinate complex cases for safe and appropriate transitions of care.
  • Communicate essential patient information to clinicians who will be initiating care.
  • Obtain necessary medical information regarding the patient’s health status, current medications and goals of care from appropriate sources.
  • Assist in development of clinical practice guidelines/standards in support of quality care.
  • Assess the patient’s and family caregiver’s needs and coordinates appropriate services as required for patient’s transition to hospice or palliative Care.
  • Assist in all facets of care coordination for referrals

 

Other Responsibilities

  • May coordinate additional services with palliative care team to assist client and family during any transition.
  • Communicate with the Clinical Managers to determine staffing
  • Develop a plan of care in collaboration with the primary care team based on his/her treatment plan that maximizes the health potential as part of a patient’s care within the continuum.
  • Provide disease management instruction and education to patients and their families.
  • Assist with Insurance eligibility and authorization process, when appropriate.
  • May assist with obtaining physician orders as required.
  • Assist in the inputting of data of palliative or hospice referrals
Qualifications:

Skills

  • Ability to market and deal tactfully with customers and the community.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or Agency requirements and is in good working order.
  • Excellent observation, verbal and written communication skills.
  • Familiar with MS Word, Outlook and Excel.
