Clinical Intake/Referral Specialist LPN

Position Overview:

The Clinical Intake/Referral Specialist is responsible for managing the intake process for patients needing home health care services. This role involves gathering information from health care providers, ensuring smooth administrative processes, and ensuring that all necessary documentation is complete before patients begin treatment or care.

Schedule: Monday-Friday, 9am-5pm

Position Type: Full Time
Essential Duties & Responsibilities:
    • Assists referral sources in evaluating service needs of client/family.
    • Processes detailed information from referral source prior to initial home assessment to enhance a smooth transition to home care.
    • Evaluates crisis calls for immediate intervention as needed.
    • Interprets Agency policy and capacity to meet requests for services.
    • Seeks and maintains current operating knowledge of community resources.
    • Communicates information received in intake to clinical areas.
    • Keeps Manager informed of volume and flow of referrals to ensure Agency’s ability to respond responsibly to service requests.
    • Has working knowledge of Medicare/Medicaid Private Insurance as it relates to home care benefit.
    • Participates in the orientation program for new staff.
    • Any other duties/responsibilities assigned by supervisor.
    • Documents any patient and or family concerns or complaints on incident reports.

 

On Call Responsibilities:

  • Answer all calls from answering service.
  • Take referrals and MD orders/lab results during non-business hours and relay information to appropriate department in morning.
  • Triage patient calls to visiting on call nurse, hospital or notifies day supervisor of problems and need for the next day visit.
  • Documents all calls on log and the status of the call.
  • Documents on clinical narrative any nursing information or teaching done to patients.
  • Any other tasks assigned by supervisor.
Qualifications:
  • Current Driver’s License

 

  • Education:
    • Graduate of NLN CNEA accredited LPN program
    • Current Massachusetts LPN license

 

  • Experience:
    • Two years of progressive experience within the field of home health care preferred.

 

  • Skills:
    • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
    • Must be familiar with MS Word, Outlook and Excel.
