Clinical Intake/Referral Specialist LPN/Medical Asistant

Position Overview:

All Care VNA has a full-time LPN/Medical Assistant position available within our office as a Clinical Intake/Referral Specialist. This position is 9:00 am to 5:00 pm Tuesday thru Saturday, working in our state of the art office 4 days a week and working 1 day remote. In this role you are the primary contact for referrals sources via Electronic platforms, fax, phone or from our Business Development Liaisons. You will review and accept Clinical referrals for all services lines and case conference with clinical managers and liaisons regarding complex care cases. You will assist in the collection of clinical information to accurately complete referral documentation/ and to help facilitate the admission process. Must have knowledge of medical terminology, excellent Customer Service skills, data entry skills, and the ability to multi- task.

Essential Duties & Responsibilities:

ESSENTIAL DUTIES AND RESPONSIBILITES

  • Respond to all calls from potential clients and referral sources. Provide excellent communication when promoting services to potential clients.
  • Conduct intakes for all clients and accurately enter data into the electronic medical record (EMR) system.
  • Reviews authorization within the EMR for accuracy and process all required documentation associated with the authorization.
  • Discharge clients within the EMR when services are terminated
  • Enter, monitor and track all service orders within the EMR
  • Contact other agency staff and outside institutions to obtain information and make appointments, etc.
  • Provides reporting statistics, other record keeping and administrative work as requested
  • Continually display excellent customer service skills
Qualifications:
  • Highly proficient with Microsoft Office applications (i.e. MS Word, Excel, Outlook, etc) and web browsers.
  • Ability to work independently and exercise independent judgment
  • Excellent organizational skills, including strong attention to detail and the ability to manage time effectively.
  • Effective interpersonal skills to interact appropriately with all levels of staff and external contacts.
  • Strong problem solving skills.
  • Proven ability to maintain strict confidentiality of all personal/health sensitive information.
