ADULT FOSTER CARE
All Care’s Adult Foster Care (AFC) Program supports caregivers and the older adults and individuals with disabilities who rely on them for assistance with daily living activities. Caregivers receive a generous stipend, customized care plan, training, and support. Care recipients benefit from personalized care and the ability to remain at home.

How Does Adult Foster Care Work?
All Care’s CARF-accredited AFC program is an alternative to nursing home or assisted living placement. In-home family caregivers provide meals, companionship, personal care assistance, and 24-hour supervision to program participants. Caregivers receive a tax-free stipend which can help to ease the financial burden of caring for the individual. The AFC program provides monthly nursing and care manager visits where both client and caregiver receive the support and resources they need to share a safe, happy, fulfilling life.
Caregivers are often family members, however, a prior relationship with the program participant is not required. Caregiver cannot be a spouse or legal guardian. Other program restrictions may apply.
Client Eligibility
- MassHealth eligible or those with Commonwealth Care Alliance, Fallon, Senior Whole Health, or Tufts Health insurance
- 16 years of age or older
- Unable to live alone due to a medical, physical, cognitive, or mental condition
- Must have qualified caregiver in the home
Caregiver Eligibility
- 18 years of age or older
- Must pass CORI
- Must live with client
- Cannot be a spouse
Benefits
- Tax-free caregiver stipend up to $1,612/month
- Caregiver support and health coaching
- Visits from an RN and Case Manager
- Coordinating social services including referrals to Adult Day Health programs and respite care
