Adult Foster Care RN

All Care is seeking a compassionate and detail-oriented registered nurse to conduct home visits for Adult Foster Care members. The Adult Foster Care RN will assess, monitor, and evaluate the health conditions of members in their home environments, providing professional medical care to clients and support to caregivers to ensure optimal health outcomes.

Work Schedule: Monday-Friday, 9am-5pm

Work Location: On the road in assigned territory, including East Boston and surrounding communities

• Assess, monitor, and evaluate the health conditions of clients in their home environments, providing professional medical care and support to ensure optimal health outcomes
• Perform comprehensive assessments of clients’ health conditions during home visits
• Develop and implement personalized care plans based on client evaluations
• Educate clients and their caregivers about health management and treatment plans
• Collaborate with other health care professionals to coordinate patient care and referrals
• Maintain accurate and up-to-date medical records and documentation

• Valid Registered Nurse (RN) license
• Strong clinical assessment skills and ability to work independently
• Excellent communication and interpersonal skills
• Proficient in using electronic health records (EHR) systems
• Valid driver’s license and reliable transportation for home visits
