$2,500 SIGN-ON BONUS

Schedule: Monday-Friday, 8am-4p

Work Location: On the road in assigned territory, including Newton and surrounding communities

Position Type: Full Time
Essential Duties & Responsibilities:
  • Practice independently in the community as part of an interdisciplinary care team.
  • Conduct comprehensive assessment of patients by determining patient needs, develops courses of action including personalized treatment plan and exceptional outcomes.
  • Constantly evaluate evolving patient needs and respond with plan of care adjustments.
  • Documents all care provided and maintain records per All Care policy and standards.
  • Participates in All Care team meetings, including but not limited to, interdisciplinary team meetings, in-service and training programs, rehab services meetings and caseload reviews.
  • Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient.
Qualifications:

Valid driver’s license required.

  • Education:
    • Bachelor’s degree in Nursing (BSN) or Associate’s degree in Nursing (ADN) from an accredited program.
    • Current and valid Registered Nurse (RN) licensure in Massachusetts.
  • Experience:
    • Previous nursing experience in a hospital, clinical, or home health care setting is preferred.
    • Experience in specialty areas such as wound care, chronic disease management, or geriatric care preferred.
  • Skills:
    • Strong clinical assessment and critical thinking skills.
    • Ability to work independently and manage patient care in an unstructured, home-based environment.
    • Proficiency in using Electronic Health Records (EHR) and other health care documentation systems.
    • Knowledge of home health care regulations, care protocols, and best practices.
    • Excellent communication, interpersonal, and organizational skills.
