All Care Hospice is seeking a dedicated Hospice Nurse Practitioner (HNP) to provide high-quality, patient‑centered care in collaboration with our Hospice Medical Director and interdisciplinary team. This is a part-time, 24-hour/week position working Tuesday, Wednesday, and Thursday.

About the Role

As a Hospice Nurse Practitioner, you will support the clinical care of hospice patients through face‑to‑face visits, eligibility assessments, symptom management, and collaborative care planning. You will work closely with patients, families, physicians, and the IDT to ensure exceptional, compassionate end‑of‑life care.

What You’ll Do

  • Conduct Hospice Face-to-Face encounters for initial and ongoing eligibility
  • Perform patient assessments, symptom management, and palliative interventions
  • Serve as attending provider upon patient request
  • Collaborate with attending physicians, consulting physicians, and Hospice Medical Director
  • Participate in family meetings and interdisciplinary team discussions
  • Educate patients, caregivers, and staff on pain and symptom management
  • Complete required documentation and maintain accurate medical records
  • Support agency growth through education and community-facing activities
  • Participate in QI initiatives, staff orientations, and peer review
  • Rotate in on‑call responsibilities with the medical team

What We’re Looking For

  • Graduate of an accredited nursing program; Master’s degree preferred
  • Active NP License in Massachusetts
  • Controlled Substance License
  • 1–2 years recent clinical experience; hospice or palliative experience preferred
  • Strong communication, clinical assessment, and documentation skills
  • Valid driver’s license and reliable transportation
  • Proficiency in MS Word, Outlook, and Excel
