Adult Foster Care Case Manager

Position Overview:

All Care is seeking a Case Manager for our Adult Foster Care (AFC) program. The Case Manager is responsible for coordinating and monitoring services for individuals enrolled in the AFC program who require assistance with activities of daily living. The AFC Case Manager ensures that participants receive safe, high-quality, person-centered care in a home-based setting while maintaining compliance with state and program regulations.

Work Schedule: Monday-Friday

Work Location: On the road in assigned territory, including Danvers and surrounding communities.

Essential Duties & Responsibilities:
  • Conduct initial and ongoing assessments of AFC clients’ physical, psychosocial, and environmental needs
  • Develop and update individualized service plans in collaboration with members, caregivers, and interdisciplinary team members
  • Perform regular home visits to monitor member health, safety, and caregiver performance
  • Provide education and support to caregivers regarding care techniques, documentation, and program requirements
  • Coordinate services with health care providers, community resources, and other support systems
  • Monitor and document changes in member condition and report concerns appropriately
  • Ensure compliance with state regulations, Medicaid guidelines, and AFC program standards
  • Maintain accurate, timely, and complete documentation in electronic medical records
  • Participate in team meetings, case conferences, and quality improvement activities
  • Advocate for members’ rights, preferences, and overall well-being
Qualifications:
  • Valid driver’s license and reliable transportation
  • Bachelor’s degree in Social Work, Nursing, Human Services, or related field
  • Current licensure (e.g., LSW, RN, or equivalent) preferred
  • Minimum of two years of experience in case management, home care, or working with elderly/disabled populations
  • Knowledge of Medicaid programs and home- and community-based services preferred
  • Strong assessment, organizational, and problem-solving skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and manage a caseload
