- Refers patients to external agencies or support groups for additional help, such as mental health services, financial assistance programs, and/or legal aid.
- Acts as an advocate for patient’s rights and needs, ensuring that they receive the appropriate care and services.
- Helps patients understand their rights within the health care system, such as insurance coverage or benefits, and works to remove barriers to care.
- Communicates regularly with nurses, physical therapists, occupational therapists, doctors, and other health care professionals to ensure a coordinated approach to patient care.
