Schedule: Monday-Friday

Work Location: On the road in assigned territory, including Cambridge and surrounding communities

Position Type: Full Time
Essential Duties & Responsibilities:
  • Monitor and follow patient progress and report health changes to supervising registered nurse through detailed observations, interviews, and physical/psychosocial care within LPN scope of practice.
  • Deliver personalized nursing and care management by constantly evaluating the evolving patient needs and adjusting the plan of care.
  • Carry out quality-driven and care coordination by communicating within the All Care interdisciplinary team and across providers and other resources to deliver the plan of care effectively and efficiently.
  • Use technology, including a tablet computer, phone, and other systems to coordinate and maintain accurate and up-to-date electronic medical records (EMRs) for patients in the community.
  • Documents all care provided and maintain records per All Care policy and standards.
  • Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient.
  • Use agility and empathy to deliver continuity of patient care including coordinating supplies and providing education.
  • Participates in All Care team meetings, including but not limited to, interdisciplinary team meetings, in-service and training programs, rehab services meetings and caseload reviews.
Qualifications:

Valid driver’s license required.

  • Education:
    • Completion of an accredited Licensed Practical Nurse (LPN) program.
    • Valid LPN licensure.
  • Experience:
    • Previous experience as an LPN, especially in home health care, geriatrics, or hospice care, is preferred.
    • Experience with wound care, IV therapy, and chronic disease management is a plus.
  • Skills:
    • Strong clinical skills to perform nursing tasks, such as wound care, and monitoring vital signs.
    • Excellent communication skills to interact with patients, families, and health care providers.
    • Ability to work independently and manage patient care in a home environment with minimal supervision.
    • Proficiency in using Electronic Health Records (EHR) and medical equipment.
    • Strong problem-solving abilities and attention to detail when monitoring patient conditions and adjusting care plans.
